Semen Analysis

At the request of a local Seattle-area treating physician or ARNP, the SRM andrology center can perform a standard semen analysis.

To schedule an appointment, the physician, physician office representative or patient can call 206-301-5000 or click here to schedule a semen analysis appointment. A written report will then be mailed back to the referring physician/ARNP for further evaluation and management.

Patients are welcome to schedule themselves, however please ensure we receive the physician referral before the patient calls. Referrals forms can be faxed to 206-285-4555.
