Erectile Dysfunction
Erectile Dysfunction (ED) is defined as the persistent "inability to achieve or maintain an erection sufficient for satisfactory sexual performance." The experience of ED varies based on patients' and their partners' perceptions and expectations about erectile function and sexual performance.
ED results when there is inadequate rigidity of the erectile cylinders of the penis. This may be due to the lack of nerve signals to initiate blood flow into the penis, impaired ability of arteries to deliver blood flow, ineffective closure of veins required for appropriate storage of blood within the erect penis, psychological factors, and hormonal deficiencies. A mixed etiology involving both organic (physical causes) and psychological factors is often present. Secondary problems related to erectile dysfunction can include loss of sexual desire (loss of libido), premature ejaculation, or inability to reach orgasm.
The last two decades has seen a dramatic increase in our understanding of the causes of and treatments for ED. Evaluation of ED is best performed by a Urologist and includes a thorough history, physical examination, and blood tests. In some cases, additional testing of penile blood flow may be recommended.
The treatment of ED includes both medications and surgery is tailored to the needs and desires of each individual patient.
